The Community Anticoagulation Therapy (CAT) Clinic facilitates management of patients on warfarin and promotes the continual improvement of healthcare within our community. The main goal of the CAT clinic is to provide patients with a better quality of service by connecting healthcare providers at a new level of communication and implementing a process for quality assurance. The clinic is staffed by a Registered Nurse with special knowledge and training in serving people who are taking Coumadin/warfarin. The CAT Clinic nurse works closely with other healthcare professionals to track and manage patients Coumadin/ warfarin intake.

How do I refer a patient?
We have found that patients who have successful experiences at the CAT Clinic display the following characteristics. Please consider these traits when referring patients to the CAT Clinic:
Eligibility Criteria

Patients agree to be compliant with instructions provided from the CAT Clinic program members, including appointments for PT/INR checks.

Patients or family members have the capacity to understand the patient's condition and implications of anticoagulant therapy.

Patients are willing to be active participants in their health maintenance.

Patients are able to travel to and from clinic appointments and/or the lab.

Patients are accessible by telephone.

Patients have a documented need for anticoagulant therapy.
Referral Process
If after reviewing the list above, you feel that your patient will have a successful experience with us, you may begin the referral process by completing a "CAT Clinic Referral Form." Please submit this form with the requested medical record information. The CAT Clinic Nurse Coordinator will review the form, and upon approval will contact the patient to set up an enrollment visit. Please remember that the referring physician is responsible for dosing warfarin until the patient's first appointment at the clinic. Once the patient is enrolled in our clinic, we will manage their Coumadin/warfarin therapy and keep you informed of their progress.
